I am taking GRE for India and would like to submit certificate of eligibility of Anxiety. I am struggling to understand the format of the documentation as multiple doctors I have reached out to are not sure about the same. If anyone is aware of the particulars, request your support.

Yes. Infact I have realised that I can apply for accommodation only after going through this blog. My pain point is that every doctor I have consulted in the past few days is asking me for a sample of the documentation as they have never issued anything like this in past.
The doctor should already have experience writing letters that confirm that someone has this or that disability. In this case, they just need to do it in a way that satisfies ETS’ requirements (Psychiatric Disabilities Documentation Guidelines | ETS) - have them read that fully. You should also consider writing a personal statement - the link explains how you can do that as well.
