I got 30 combinations for this and the answer for this is 45. I think the question doesn’t clearly mention that customers can also have 2 main course.

Adding all these 4 gives you 30. To reach 45, you would need to consider the options where customers could chose 2 main course too which will give you 4 more combinations but question doesn’t clearly specify if we should also consider two main course it says must have and not can have.
