I am willing to maintain an error log but don’t know how to make one. Please recommend me a template to follow. Suggestions to add columns on would be appreciated too!

Made an error log like this for Manhattan 5lb book Quant section
Column A is Chapter name
B is Question Number
C indicates if I got the question wrong (Y means yes, blank means I got it correct)
D indicates Silly Mistake
E indicates if the question is important for future reference
F indicates if I should check the solution from Manhattan answers at the end of each chapter.
Don’t know if this would be useful for you, but it definitely helped me out!
Also, I took a print out of this, and re-solved all of the questions listed. It helps with identifying & rectifying your mistakes.
