Serious conceptual dearth in algebra and arithmetic as well…See Greg’s videos on concept series and do the dedicated concept videos. Solve Manhattan 5lb (For Algebra and arithmetic). Review the ones you get wrong. Try solving same problem in different methods or see if you can approach it via different paths. This has helped me greatly. Hope this helps.
Gregmat has many practice resources. Go through the quant practice videos where Greg interacts with students. You’d be amazed to see so many bright students come with such elegant solutions. If you are able to solve the medium and majority of the tough ones you are good to go.
Also, need to apply new techniques and ditch the traditional techniques which you did in high school. It is not easy solving 20 problems in 35 minutes. You, need a hell lot of practice. GRE Math is not easy.

To me I think you still have a shaky foundation - the quant concept series could help.

After you are done with that, solve the next list on a daily basis
For QC questions, do what Greg says, try to make A and B equal and then do a different iteration for showing them unequal. That saves a lot of time.
