Differentiating block of four and contrast

I have a concern in TC: how could we differentiate between a sentence that has a block of 4 and when a sentence has two parts and the relation between the two parts is contrast but the contrast has existed already?
For example ( the first sentence in the picture )

With Block of Four, you will find four components (two pairs) where one of them is the answer. The two pairs can either be synonyms or antonyms.
For example, eating vegetables is good for your health. Therefore, eating ____ food is bad.
With two contrasting ideas, you may or may not have four components regardless of contrasting words such as, even though, however, despite, etc.
